CERP information for IBCLCs

Each chapter of Spanish for Breastfeeding Support has been approved as an independent study module.  International Board Certified Lactation Consultants may earn L CERPs for completing these modules.  CERP forms are provided in the book and can also be downloaded from this page. 

To earn CERPs for completing some or all of the chapters in this book, fill out the following CERP forms for each chapter you complete:

• self assessment questionnaire form (complete only once if submitting several chapters
• answer sheet (CERP post-test)
• self-directed evaluation

Instructions:

1. Jot down the time you start and end each chapter module, including time to read through chapter, complete the exercise, and take the post-test.

Put this information on the evaluation sheet.

 

2. Read the module.

 

3. On the Self-Assessment Questionnaire Form, enter your answers to the Post-Test questions.  Complete the rest of the information on the page.

 4. Check your answers using the Answer Key. If you missed any answers, reread that section in the module. Retake the test if you missed more than 3 questions. You must answer 70% correctly to get continuing education credit for this chapter module.

5. Complete the Self-Directed Evaluation.

 

6. For each chapter module you complete, send your completed Self-Assessment Questionnaire Form, Self-Directed Evaluation, and check for $20.00 or credit card information to:  Hale Publishing, 1712 N. Forest St., Amarillo, TX 79106

7. You will be sent a Certificate of Successful Completion.  Please keep this certificate on file.
